Patient Information
Before your appointment:
We will contact you 2-3 days before your scheduled appointment date to confirm/communicate any instructions in preparation of your appointment
Please let us know a week in advance if you need to cancel or reschedule your appointment
If requiring IV sedation:
VERY IMPORTANT: if you require IV sedation, have nothing to eat 8 hours prior to the scheduled procedure time; from this point clear liquids are allowed up to 3 hours prior. Then have nothing to drink, including water for the remaining 3 hours before your appointment. For example, if your procedure was scheduled at 2 pm then you cannot eat after 6 am and drink clear liquids only until 11 am, having nothing in your stomach after 11 am.
Wear a short-sleeved shirt that you feel comfortable in
Please arrange for a responsible adult to drive you home and to keep you company for the day
**If you are not having ID sedation, you will not need a driver to drive you home, or to fast 8 hours prior

Financial Information
We will attempt to accomodate direct direct billing with insurance company’s cooperation. Some insurance companies will not give us specifics, therefore payment for each service rendered is due in full at the time of your appointment. While we are happy to provide our patients with an estimate of procedural costs, please remember that you are responsible for the complete cost of your appointment, regardless of your level of insurance coverage.
The finances of your care will be comprehensively discussed with you during your consultation appointment. Our knowledgeable staff is here to assist you in making your payment as smooth as possible by helping you submit your dental insurance claims from our clinic at the time of your appointment.
To ensure you have the quickest reimbursement possible, please bring your complete insurance information with you on the day of your appointment. This includes: your insurance provider name, policy number (sometimes called group/plan/contract) number and your certificate/ID number. Also we need the name of your plan holder and their birth date. For example Sunlife plan holder John Doe birth date January 1st 1960 (for patient Susan Doe) policy #023246 and certificate #001498283.
