At Omfs Inc we strive to ensure our patients receive the best possible outcome for their health through providing the highest standards of care, safety, and comfort.
When you arrive to our office, you will be welcomed by our reception staff who will register you as a patient. Please complete your online medical history fully, including any medications, or supplements you take and their dosage, as these factors may affect your treatment and recovery. If you have a long list of medications, please leave your pharmacy name and phaarmacy location on the health form and we will have them fax us a list of your medications. During your initial consultation appointment, Dr Ross will review your dental/medical history and perform an oral examination and review X-rays to comprehensively outline all treatment options and find a solution that works best for you. Following consultation, our staff will schedule you for any necessary procedures. However, some patients prefer the procedure done at the time of the consultation. Our staff needs to know ahead to be able to book appropriately for time.
Please be aware that you must have a driver to drive you home if you are being sedated (IV sedation only). All patients under 19 should be accompanied by a parent or guardian at the consultation.
Payment and Insurance:
We accept most insurance plans.
For your convenience, please be sure to bring your insurance information with you on the day of your appointment so that we may submit a claim on your behalf and you are reimbursed quickly.
All payment is due the day of the appointment.
